Elderly and Disabled Program

SEAT offers a discounted fare, transportation program for those passengers that are 65 years of age or have a qualifying disability. Passengers may complete the Elderly and Disabled Application for approval of the program. If you cannot print the application below, you may call our Transit Center and request a copy of the application be mailed to you.

Passengers approved for the Elderly or Disabled Transportation Program may take a Personal Care Attendant with them free of charge. Please be sure to have your PCA’s name for customer service staff when scheduling a reservation.

If you are 65 years of age, the following must be completed and returned:

  • SEAT’s Elderly and Disabled Application completed and signed (section 1 and section 3)
  • Proof of age (driver’s license, birth certificate or State Issued ID)
  • Photo ID

If you are not 65 years of age and are requesting to be approved for the Elderly or Disabled Program, you must complete and provide the following information:

  • SEAT’s Elderly and Disabled Application completed and signed (section 1 and section 3)
  • Photo ID
  • Certification of disability by a medical provider (section 2)

OR

  • Red, White, and Blue Medicare Card
  • Copy of Social Security Benefit statement saying you are certified by the Social Security Department as disabled

Passengers may be given a 30-day grace period to return the application to SEAT if they are 65 years old or have a qualifying disability. If the completed application and proof of age or disability is not provided to SEAT during the 30-day grace period, passengers may not be eligible for discounted fares until the required information is provided to SEAT.
